Chicago (CBS) – Three more cash prizes worth $ 100,000 were announced in the Illinois vaccine lottery on Monday, with the winners in Chicago, Bervin and Jolie.
The Illinois Department of Public Health announced the winners Monday afternoon by calling 312-814-3524 and / or email DPH.email@example.com.
Winners can remain unknown and have seven days to receive their prize. The state will announce all the winners who wish to go public for eight days.
Winners will not be asked for any personal information on the first phone call or notification via email. They have seven days to fill out their authorization form, sign it and send it to the state to receive the awards. So the Illinois lottery will help them in the lawsuit process.
The state’s All-Winning Lottery is open to anyone 12 years of age and older who has at least one dose of COVID-19 vaccine in Illinois.
Over the summer, the Illinois lottery named after vaccinated adults, with cash prizes ranging from $ 100,000 to $ 1 million. For children ages 12 to 17 who have had at least one dose of the vaccine, they can get a Bright Start College savings plan of $ 150,000 each – the equivalent of a full trip to a four-year public university in Illinois.
The state announced the first cash prize for the vaccine lottery on July 8, along with three $ 150,000 scholarships for youth ages 12 to 17. The first three winners of the $ 100,000 cash prize were announced on July 12th.
Illinois will offer a total of three cash prizes of $ 1, a $ 100,000 prize pool and twenty $ 150,000 scholarships.
